Medical Records Management
We assist in obtaining and delivering any/all medical records in order to provide the best quality of care.
In addition to coordinating between medical teams and insurance providers, we supervise, facilitate, monitor and assist our patients with their medical care.
We coordinate care between all treatment team providers, which may include, but is not limited to, physicians, therapist and rehabilitation specialists.
Comprehensive Medical History
We complete a detailed medical evaluation and records review in order to ensure the client’s medical care is appropriate for their specific medical needs.
Supervision of Treatment Plans
We assist with the facilitation of treatment plans, as established by our patients’ physicians.
We evaluate ongoing healthcare needs, provide counsel and offer support to our clients and their families.
We attend all injury related medical appointments and evaluations to ensure our patients receive the highest quality care.
Hospital Discharge Planning
We coordinate with the hospital discharge planner to be sure all medical needs and equipment arrangements are made prior to discharge.
We facilitate both skilled and unskilled care services in the home or on an outpatient basis.
Medical Supplies/Equipment Assistance
We facilitate all medical supplies and equipment as ordered by the treating physicians.
As part of our case management services, we will communicate regularly with your insurance carriers to make sure you are receiving all the eligible benefits available to you in your insurance policy(s).
Claims Processing (According to Plan’s benefits)
We administrate claims by communicating with insurance companies to determine the benefits under each client’s policy/policies.
We coordinate multiple insurance policies in order to be sure the proper medical billing order is in place.
We communicate with the claims adjusters to determine the eligibility and authorization of all medical needs as ordered by the treating physician.
Home Modification Management
We ensure all recommended home modifications are authorized by insurance providers, as well as assist with any/all documentation requirements needed by the insurance company.
